Document to support weights of damage/missing articles

Document to support repair cost

Photos including a good overview of the item(s) damaged and the specific parts that are damaged

Please keep any items that are damaged and their packaging in case an inspection is needed.

Please keep copies of all pertinent information with your copy of the claim form so that if a repair firm is assigned, they can review the documents. This should include the delivery receipt, proof of purchase, and any other information that may be helpful.

All claims must be received by the carrier within nine months from the delivery date or they will not be accepted for payment.

Please explain under the remarks an absence of any documents that are required with your claim.
